Tutoring Services Contract

1. This agreement is made between _______________________ and The Learning Habitat

for the provision of tutoring Services for ___________________________________ for

the abovementioned period (Please note we are closed March Break and Christmas).

2. I agree to pay the following cost (payable on the first week of each month) for online

classes via Zoom or similar online platform. The cost will include holidays, March break,

and Christmas break, as well as 5- week months.

⃝ $175 per month for one hour each week (one on one)

⃝ Other $_________ for ____________per week (one on one/2 students per class)

3. I agree to provide a minimum of one (1) month notice (or payment in lieu of notice) in

the event that I need to withdraw tutoring services during the school year.

4. I agree that in the event of the absence of my child, rescheduling will be discretionary

on the part of The Learning Habitat and dependent on availability.

5. In the event The Learning Habitat is unable to provide services for a scheduled class, I

agree to the class (es) being rescheduled at a mutually convenient date and time.
